Join Our Team: Licensed Vocational Nurse (LVN)


We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team. This role is focused on supporting residents and upholding their rights in our housing programs. You will be part of a collaborative team environment and make a meaningful impact on the lives of individuals and families striving for stability.


Job Summary


The LVN will provide direct nursing care, medication monitoring, monitor residents’ health conditions, and ensure compliance with medical protocols. Additionally, the LVN will advocate for participants' rights, provide health education, and coordinate care for individuals with complex medical needs.


Essential Duties & Responsibilities


  • Address any concerns related to staff performance and collaborate on professional development.
  • Work as part of an interdisciplinary team to provide comprehensive, integrated care to individuals experiencing homelessness, mental illness and substance addiction
  • Meet with new clients within 24 hours to complete medical assessment of intake.
  • Ensure that clients prescribed medications have arrived.
  • Provide basic nursing services as needed, including injections, to clients residing in interim supportive housing.
  • Remain current in knowledge of Medi-Cal benefits and healthcare systems navigation
  • Coordinate PCP appointments or other medical follow up appointments
  • Provide medical case management/medication management to individuals including but not limited to medication monitoring, patient teaching, and supportive services to increase compliance with medication regimens
  • Along with Program Manager, maintain medication logs for clients.
  • Coordinate with pharmacy to ensure client medication is delivered on time
  • Document and chart all services provided in appropriate charting form with accuracy and in real time
  • Collaborate and communicate effectively with internal and external programs on behalf of clients to ensure continuity of care
  • Conduct regular audits of medical charts to ensure accuracy, completeness, and compliance with regulations.
  • Conduct regular health education and workshops for participants on topics such as medication management, hygiene, chronic disease management, mental health, substance abuse awareness, nutrition, medication management, and sexual health.
  • Provide crisis intervention assistance when needed.
  • Triage to appropriate community agency in an emergency (i.e., police, paramedics, fire department)
  • Maintain open communication with direct supervisor and leadership team
  • Participate in weekly staff meetings and weekly individual supervision
  • Ensure that medical services are reflective of the agency’s treatment model, which emphasizes member choice, harm reduction, trauma-informed care, and psycho-social rehabilitation
  • Always maintain confidentiality per HIPAA and all applicable guidelines
  • Other duties as assigned.

Compensation: $32.00 - $35.00 per hour
